I'd also like to nominate that first appearance of that Zygon as Sarah is talking to the Doctor over the phone.
I pooped my undies watching that on first broadcast and leapt off the floor, scuttling behind my mother's back for comfort. Fat lot of good she was as she laughed away.

Bare in mind i was 3 years old, but at the end of episode 1 of Rememberance of the Daleks when the Dalek was hovering up the stairs shouting ''exterminate'' scared the living hell out of me and has stuck with me till this day. The look on the Doctor's face made it more terrifying!

In Blink, when Larry had to keep an eye on the Weeping Angel. He blinked and it was in his face...

William Hartnells first encounter with the Daleks, I was not very old and it just glided in ...no noise no heavy breathing it just glided in...Ooh I wailed louder than any police car.LOL

I don't find Doctor Who scary in the slightest

In my opinion, the very first Daleks are still the scariest. I used to find them scary, but still reckon they're creepy...and I wish I'd got into Doctor Who when I was young enough to find them terrifying xD
I nearly cried when I saw Billy Hartnell steal the TARDIS in 7-13.
